You are hereby notified that a petition under oath has been filed in the above-styled court for the termination of your parental rights as to A.B a female child born on the 8th day of April, 2009, in Durham County, North Carolina and for placement of the child with the Florida Department of Children and Families for subsequent adoption, and you are hereby commanded to be and appear before the Honorable Stephen E. Toner, Jr. of the Circuit Court or any judge assigned to hear the above cause, at the Termination of Parental Rights Advisory Hearing on January 8, 2024, at 9:00 AM at the Hernando County Judicial Center, 20 North Main Street, Brooksville, Florida 34601, 4th Floor, Court Room E.
YOU MUST PERSONALLY APPEAR ON THE DATE AND TIME SPECIFIED.
FAILURE TO PERSONALLY APPEAR AT THIS HEARING FOR TERMINATION OF PARENTAL RIGHTS CONSTITUTES CONSENT TO THE TERMINATION OF PARENTAL RIGHTS TO THIS CHILD. IF YOU FAIL TO APPEAR ON THE DATE AND TIME SPECIFIED, YOU MAY LOSE ALL LEGAL RIGHTS TO THE CHILD NAMED IN THE PETITION.
YOU ARE ENTITLED TO HAVE AN ATTORNEY PRESENT TO REPRESENT YOU IN THIS MATTER. IF YOU WANT AN ATTORNEY, BUT ARE UNABLE TO AFFORD ONE, YOU MUST NOTIFY THE COURT, AND THE COURT WILL DETERMINE WHETHER YOU QUALIFY FOR AN ATTORNEY TO BE APPOINTED TO REPRESENT YOU IN THIS MATTER.
