NOTICE
Effective September 20, 2024, PT Solutions Holdings, LLC purchased certain assets belonging to, and is now providing physical therapy services to the public at the former location of Physical Therapy Associates of Brooksville, Inc. d/b/a Physical Therapy Services of Spring Hill. All patient medical records of Physical Therapy Associates of Brooksville, Inc. d/b/a Physical Therapy Services of Spring Hill, relating to services rendered prior to September 20, 2024 have been transferred to PT Solutions Holdings, LLC and will continue to be available to patients at the prior location of Physical Therapy Associates of Brooksville, Inc. d/b/a Physical Therapy Services of Spring Hill, 3247 Commercial Way, Spring Hill, Florida 34606.
All former patients of Physical Therapy Associates of Brooksville, Inc. d/b/a Physical Therapy Services of Spring Hill may obtain copies of their medical records by sending a written request/authorization to the following address:
