Effective September 20, 2024, PT Solutions Holdings, LLC purchased certain assets belonging to, and is now providing physical therapy services to the public at the former location of Physical Therapy Services of Brooksville, Inc. All patient medical records of Physical Therapy Services of Brooksville, Inc. relating to services rendered prior to September 20, 2024 have been transferred to PT Solutions Holdings, LLC and will continue to be available to patients at the prior location of Physical Therapy Services of Brooksville, Inc., 20195 Cortez Blvd, Brooksville, Florida 34601.
All former patients of Physical Therapy Services of Brooksville, Inc. may obtain copies of their medical records by sending a written request/authorization to the following address:
