Savage, Esq., whose address is 412 East Madison Street, Suite 815, Tampa, FL 33602 no later than 28 days after the first publication of this Notice, and file with the clerk of this Court, Doug Chorvat, Jr., whose address is 20 N. Main Street, Brooksville, FL 34601, before service on Plaintiff, or immediately thereafter. If you fail to do so, a default may be entered against for the relief demanded in the declaratory judgment action.
Copies of all court documents in this case, including orders, are available at the Clerk’s office. You may review these documents upon request.
You must keep the Clerk of the Circuit Court’s office notified of your current address. Future papers in this lawsuit will be mailed to the address on record at the Clerk’s office.
