You are hereby notified that a petition under oath has been filed in the above-styled court for the termination of your parental rights to E. M. G., a male child born on the 17th day of May, 2012 in Louisville, Jefferson County, Kentucky, for placement of the child with the Hope Green and Burton Green for subsequent adoption. YOU ARE HEREBY COMMANDED to be and appear before the Honorable Stephen Toner, Judge of the Circuit Court or another judge assigned to hear the above cause, at the TERMINATION OF PARENTAL RIGHTS ADVISORY HEARING at 2:00 P.M. on October 17, 2022, at the Hernando County Courthouse, 20 N. Main Street, Brooksville, Florida 34601 in Courtroom E. You must appear on the date and time specified.
FAILURE TO PERSONALLY APPEAR AT THIS ADVISORY HEARING CONSTITUTES CONSENT TO THE TERMINATION OF PARENTAL RIGHTS OF THIS CHILD. IF YOU FAIL TO APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL LEGAL RIGHTS TO THE CHILD NAMED IN THE PETITION ATTACHED TO THIS NOTICE.
YOU ARE ENTITLED TO HAVE AN ATTORNEY PRESENT TO REPRESENT YOU IN THIS MATTER. IF YOU WANT AN ATTORNEY, BUT ARE UNABLE TO AFFORD ONE, YOU MUST NOTIFY THE COURT, AND THE COURT WILL DETERMINE WHETHER YOU QUALIFY FOR AN ATTORNEY TO BE APPOINTED TO REPRESENT YOU IN THIS MATTER.
